CareVisor Privacy Policy

At CareVisor, we kn­ow our users care about their personal information and how that information is used and shared. We take your privacy seriously. The following Privacy Policy explains how CareVisor will use your information in regards to this App created by CareVisor.

Information we obtain from you

CareVisor obtains information directly from you when you register to use the Mobile App. This information helps CareVisor improve our services and allows App users to set up a user account and profile that can be used to interact with the CareVisor services through this Mobile App.

User-provided Information

You are required to download and register with CareVisor to use the Mobile app. In going through this process, you will generally provide the following types of information:

  1. Contact details: Your name, contact information (such as email and home address, mobile and home phone numbers), a username, and password
  2. Demographic information: your age/date of birth
  3. Health information: diagnosed health conditions, the medications and supplements you take regularly, your health care plan, your behaviors (e.g., whether you smoke), and biometric information (e.g., height, weight, peak flow)
  4. Contact information for the friends and family members you select for us to contact in case you need help as well as for your physicians
  5. Other information, such as your daily routine

Personally Identifiable Information: Personally identifiable information (PII) means any information that may be used to identify an individual, including, but not limited to, a first and last name, email address, a home, postal or other physical address, and phone number. We collect PII from you when you register and use the CareVisor Mobile App.

Health and medical information: in order for CareVisor to provide its services we may collect some medical and health related information including your physician’s name and address, prescriptions and non-prescription drug information, medical diagnostics, and medical history.

CareVisor’s Mobile App will use and disclose PII and protected health information in a manner that is consistent with applicable requirements under the Health Insurance Portability and Accountability Act of 1996 and the rules and regulations promulgated thereunder (“HIPAA”). How we use your information

CareVisor will use your information to exchange communications and data with you. We may also use the information you provide us to contact you from time to time to provide you with required notices and other important information.

Information collected automatically

The CareVisor Mobile App may also collect certain information automatically, including, but not limited to, the type of mobile device you use, your mobile device’s unique device ID, the IP address of your mobile device, your mobile operating system, the type of mobile Internet browsers you use, and information about the way you use the CareVisor Mobile App.

Sharing of personal information

We will neither rent nor sell your User-Provided Information to anyone, except as in the ways that are described in this Privacy Policy. Information Shared to Perform Services: we may share your information with your doctors or other healthcare professionals involved in your care or treatment. Information Shared with Service Providers: we may share your information with third party business associates that we contract with to perform various services (e.g., server hosting). The information we share with business associates is limited to that information needed to provide the contracted services. Also, each business associate is required under our contracts with them to protect your personal and medical information, and not to use it for any reason except as necessary to provide that contracted services. CareVisor may use the information you provide: As required by law, such as to comply with a subpoena, or similar legal process. When we believe in good faith that disclosure is necessary to protect our rights, protect your safety, or the safety of others or to investigate fraud. If we are involved in a merger, acquisition, or sale of all or a portion of its assets, you will be notified via email and/or a prominent notice on our web site of any change in ownership or uses of this information, as well as any choices you may have regarding this information. We may de-identify the information you provide, so that it cannot be used to re-identify you. We may disclose such de-identified information to third parties who perform research, science and analytics to help us understand the relative effectiveness of our services. We may disclose such de-identified information to third parties who perform accreditation services. If you are a California resident, California Civil Code § 1798.83 permits you to request certain information regarding the disclosure of your personal information to third parties for direct marketing purposes. Further, CareVisor protects the privacy and security of your protected health information (as defined by the Health Insurance Portability and Accountability Act of 1996 and the rules and regulations promulgated thereunder (“HIPAA”)) for certain CareVisor interactions subject to HIPAA regulations.

Automatic data collection and analytics

We may work with analytics companies to help us understand how the CareVisor App is used, such as the frequency and duration of usage. Analytics companies use some of the information collected by us, including, but not limited to, the unique identification ID of your mobile device. To protect the anonymity of this information, we use an encryption technology to help ensure that these third parties cannot identify you personally. These third parties may also obtain anonymous information about other applications downloaded to your mobile device, the mobile web sites you visit, your non-precise location information (e.g., your zip code), and other non-precise location information in order to help analyze features. We will not honor any Do Not Track (“DNT”) signals sent by your browser. We may also share encrypted versions of information you have provided in order to enable our partners to append other available information about you for analysis. Some third party sites also track your browsing activities when they serve you content, which enables them to tailor what they present to you. Your browser may allow you to set the DNT signal on your browser so that Web site operators know you do not want to be tracked.

What are my opt-out rights?

There are multiple opt-out options for users of the CareVisor Mobile App:
You can stop all collection of information by us easily by uninstalling the CareVisor Mobile App. You may use the standard uninstall processes as may be available with your mobile device or via the mobile application marketplace or network. You can also send your request to opt-out via email, to support@carevisor.com.

Data retention policy and how to manage information provided by you

We will retain the information you provided us for as long as you use the CareVisor Mobile App and for a reasonable time thereafter. We will retain automatically collected information for up to 6 years and thereafter we may store it in aggregate. If you would like us to delete information that you have provided via the CareVisor Mobile App, please contact us at support@carevisor.com and we will respond in a reasonable time. Please note that some or all of the information you provided through the CareVisor Mobile App may be required in order for the App to function properly.

Children

We do not use the CareVisor Mobile App to knowingly solicit data from or market to people under the age of 18. If a parent or guardian becomes aware that his or her child has provided us with information without their consent, he or she should contact us at support@carevisor.com. We will delete such information from our files within a reasonable time.

Security

We are concerned about safeguarding the confidentiality of your information. We provide physical, electronic, and procedural safeguards to protect information we process and maintain. For example, we limit access to this information to authorized employees and contractors who need to know that information in order to operate, develop or improve the CareVisor Mobile App. Your account is protected by a password for privacy and security. We endeavor to provide reasonable security for information we process and maintain, but please be aware that no security system can prevent all potential security breaches.

Changes

This Privacy Policy may be updated from time to time for any reason. We will notify you of any changes to our Privacy Statement by informing you via e-mail. You are advised to consult this Privacy Policy regularly for any changes, as continued use is deemed approval of all changes.

Your consent

By using the CareVisor Mobile App, you are consenting to our processing of your information as set forth in this Privacy Policy now and as amended by us. “Processing,” means using or touching information in any way within the CareVisor Mobile App, including, but not limited to, collecting, storing, deleting, using, combining and disclosing information, all of which activities will take place in the United States.
